Hospital to Hospital (H2H) Programs
RWHC developed the Hospital to Hospital (H2H) Program, building on a recommendation from the keynote address at Wisconsin’s 2005 Rural Health Conference that rural hospital CEOs should seek out and visit other hospitals routinely to gain insight and enhance their own organization’s performance. RWHC member hospitals interested in participating submit a brief description of the program they are willing to share with a team of visitors, and the board’s steering committee chooses from the list of nominees for two to three site visits a year. RWHC hospital CEOs are given the first opportunity to register, and each site visit is limited to 20 participants. If 20 CEOs do not sign up, participating CEOs are encouraged to bring a guest until all 20 slots are filled.

Columbus Community Hospital – Focus on Performance Improvement: A Systematic Approach to Addressing Employee Opinion via Departmental Action Plans (2 Parts)

View Part 1
View Part 2

Presenter(s): Ed Harding, President & CEO, Columbus Community Hospital

In 2004, Columbus Community Hospital (CCH) began conducting an employee opinion survey in an effort to identify and improve upon a number of organizational issues. HR Solutions, a reputable, nationally recognized survey firm conducting approximately 500 hospital surveys nation-wide was hired to complete this task. Year One (2005) results were less than stellar. On key indicators, CCH ranked at the 12th percentile nationally. Rather than wallowing in self-pity, the CCH management team recognized this as a great opportunity. A systematic approach was developed to address employee opinion via departmental Action Plans. These Plans required employee participation and buy-in. Year One action plans totaled over 170. This may sound a bit staggering, but when you rank in the 12th percentile, there is a lot to improve.

Results of the 2006 survey showed dramatic improvements from a statistical standpoint, but still not where CCH wanted to be. In fact, CCH received HR Solutions national award for the most improved hospital by moving to the 39th percentile. Through the creation of additional measurable Action Plans, CCH ranked within the "Best Practice" range (90th percentile) in almost every single indicator in 2007. In many of the areas CCH was actually the best practice nationally! Maintaining that level of performance was the focus of this past years’ survey. In 2008 CCH achieved a national ranking of 97th percentile. So how did CCH achieve this performance? Well that's what you'll find out through this H2H Program. As you can imagine, there is no panacea, no magic bullet. The Action Plans were the most significant reason because employees were heavily involved in drafting and implementing the Plans.

† "Future of Care Management"

Presenter(s): William F. Tell, Senior Director of Educational Services, The Advisory Board Company

The fragmented nature of the U.S. health care system is coming into full view as demographic and epidemiological forces create an ever-expanding pool of chronically ill patients. Progressive health systems have filled the gaps by restructuring their clinical programs around at-risk populations, investing in information technology to improve care coordination, and strengthening linkages to community providers. This presentation will explore the clinical, financial, and operational value of investments in care coordination.

"Navigating Physician Employment "

Presenter(s): Timothy Randall, The Advisory Board Company

Even hospitals that have long minimized physician employment now confront an increasing number of scenarios in which employment appears to be the only option to obtain or retain physicians for key hospital strategic priorities. Between stabilizing market access to threatened specialties, defending against proactive employment by competing hospitals, and utilizing employment contracts as last-ditch recruitment levers, the numbers—and types—of employed physicians often quickly—but steadily—add up. To avoid organizational chaos and the demonstrated financial and strategic risks of past employment debacles, relatively inexperienced hospital employers must quickly scale the employment competency learning curve. To that end, this Health Care Advisory Board presentation guides hospital leaders through key lessons in deciding when to employ physicians, as well as how to begin building an organizational structure that will promote manageability, minimize risk, and position the hospital to maximize gains from a frequently unavoidable investment.

The Integration Imperative: State of Hospital-Physician Relations circa 2008

Presenter(s): Jessica Sweeney-Platt

The Advisory Board’s State of the Union presentation for this year discusses a coalescence of forces—accelerated by the rapidly weakening economy—that will fundamentally change the shape of the health care delivery system over the next 5 to 10 years: soaring costs of health care services, continued erosion of employer sponsored coverage and rising ranks of the uninsured, growth in consumer price-sensitivity, and troubling trends in hospitals’ bad debt and elective procedural volumes. Coinciding with an election year in which health care has reached new significance on the political agenda, these factors are fueling political and market-based proposals for radical payment reform to “solve” systemic cost and quality problems and unite an ever-fragmenting delivery system

Engaging the Nurse Workforce

Over the next decade, hundreds of thousands of new nurses will enter the hospital workforce, replacing the industry's most seasoned clinicians. In general, these nurses have high expectations regarding work flexibility, pay and personal career fulfillment. This presentation focuses on enhancing levels of engagement amongst staff nurses. The research begins with findings from The Advisory Board's Nurse Engagement database, a proprietary survey of 4,500+ staff nurses. Survey results include a rank ordering of the most important attributes in driving staff nurse engagement. The bulk of the presenation presents strategies for pursuing these drivers of engagement at both the unit and institution-level. Each strategy is supported by detailed best practices drawn from exceptional nursing programs around the country.

Operational Excellence

Technological advances and the decentralization of services are also challenging "tried and true" management practices. New techniques demand greater levels of staff skill and expertise, and the geographic distribution of services requires innovative thinking on how to effectively schedule procedures, deploy staff, and ensure optimal throughput. Given these challenges, this presentation will identify exceptional scheduling practice at progressive institutions, and weigh the benefits of centralized versus decentralized scheduling models. This session will also examine ideal staffing models per modality, provide case studies of successful throughput improvements, and profile new mechanisms for reliably measuring productivity.

Facility Investment Blueprints: Infrastructure Investment Guidance for 2007

Hospitals and health systems engaged in facility projects invariably find themselves torn between the best intentions for aesthetics, capacity, future flexibility, and a host of other aims, and the real-world reality of constrained resources and conflicting advice. The Advisory Board’s Innovations Center spent the better part of two years interviewing architects and hospital administrators, reading clinical literature, visiting “best-in-class” facilities, and constructing analyses.  The end result—a series of reports on best practices for designing and building specific areas within the hospital, with a particular focus on the med-surg floor and ancillary services.

* A Practical Guideline for Hospital Management of Medical Errors

Presenter(s): Sarah E. Coyne, Attorney, Quarles & Brady, LLP

The stakes have skyrocketed for Wisconsin hospitals where medical errors are concerned.  While the hospital industry has made  great strides in error prevention, hospitals must now consider how they will  react if and when an error occurs despite all the best plans, intentions,  policies, procedures and quadruple checks. This presentation will focus  on practical steps for hospitals to take to manage all of the potential  fall-out from medical errors. Perhaps the most frightening potential  fallout is potential criminal liability. While managing the potential criminal  liability is somewhat unpredictable, this presentation will cover strategies  and advance planning for responding to inquiries from criminal investigators. This presentation will also cover practical steps for managing patient  claims and civil liability, regulatory survey and citation from state and  federal regulators, considerations for responding to media and the press (and the confidentiality concerns relating to those responses), as well as methods  of communicating effectively with your staff and with your  patients.
